- I look after the elderly and frail in the comfort of their homes, some of whom are bed or chair ridden with medical conditions such as Alzheimer's, Dementia, Parkinson’s, heart conditions, respiratory illness, medically unwell patients and those at high risk of falls.
- I help with washing and dressing, assisting with meal preparations, house work duties and outdoor walking to the local shop and cafes.
- I ensure they are clean and tidy and that their home is well looked after.
I arrange additional support like cleaners, gardeners, food shopping delivery and organise their appointments (i. e. hair salon and hospital appointments).
I assess, order and issue equipment’s such as walking mobility aids, wheelchairs, bed rails, stair rails, pressure cushions, continence pads, commodes, toilet riser, and orthotic foot wear and ensure they're used correctly and safely.
I advise and make referrals for carer support, district nurses, continence team, occupational therapist and other health care professionals.
- I ensure they are taking their medications.
- I devise and carry out exercises with patients, whether it be land based or hydrotherapy exercise at their local pool.
- I monitor and evaluate their physical, medical and cognitive state and make onwards referrals when indicated to the relevant medical teams.
- I teach and educate relatives and careers about falls prevention, managing and preventing frailty, manual handling advice (with hoists and transfer aids) and long term care of the elderly.
- I thoroughly love providing friendly care and engaging in activities or hobbies they enjoy doing, whether it be gardening, puzzles, board games, talking over coffee or simply sitting and watching TV together.
